The surgery offers an annual health check with one of our HCAs, and a review appointment with one of our practice nurses.

Annual health check with the HCA can be booked by calling the Care Navigation Team on 02380 692000 (opt 1).

The annual health check includes a blood test, blood pressure reading, height & weight checked, and a foot examination.
You will need to bring in a urine sample to the appointment.

At this appointment the HCA will book your review with a practice nurse. At the review, the nurse will discuss your results and offer you any advice needed.
