Home BP Readings Submission Form

This form includes the option to submit up to 7 Full Days of BP Readings.

Once submitted, you will be contacted later to confirm if your results are to satisfaction or not.

How to take your BP Reading at Home:

You need to be sat down with the monitor at the same level as your heart.
Take two readings at least two minutes apart.
Take readings daily, ideally in the morning and the evening for 7 days.

This form can be used once you have taken at least 5 days of consecutive readings.
