New Patient Registration

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a new patient check with one of our healthcare assistants.  You will need to complete a registration form and bring two forms of ID into the practice before our registration can be completed.  We will register you for online appointment booking and repeat medication requests at the same time. Please speak to our receptionist when you attend.

Medical treatment is available from the date of registration. 

Practice Area

Registration accepted from residents of Bishopstoke, Fair Oak, Horton Heath, areas of Colden Common and Durley.

Please note that all patients will be registered with Stokewood Surgery but you may opt to be seen in Old Anchor Surgery if the services you require are available.  These include routine GP, nurse and healthcare assistant appointments.  

Patients needing a same day/urgent appointment will be required to attend Stokewood Surgery. 

No acute medical services are available at Old Anchor Surgery.
