If your doctor has referred you to a specialist for further care you will receive your appointment information directly from the hospital by post.
To track your e-Referral you will need the following details to log into the e-Referral website:
If you've not received anything within 8 weeks, you can collect further information from Stokewood Surgery at the reception desk, which will allow you to access more details from the referral website. Or you can find the information via the NHS App (please see below).
For quick and easy access to your referral information, you can find all details of your referral by using the NHS app (referral document can be found at GP Record>Documents).
The app can also keep you updated if your referral is currently being triaged by the hospital.
