If you have recently changed your legal name, please notify Stokewood Surgery as soon as possible so we can update your records.

We cannot accept an online application to change your name, you will have to come into the surgery and bring in official documentation as proof your name has changed. Your official documentation will be a Divorce certificate, Marriage certificate or Deed Poll certificate.
