You are no longer required to bring in Identification to approve your registration request.

Just complete the online registration forms below, we should register you within 5 working days of receiving your details.

Please book a new patient check appointment so that screening programme information and medication details can be verified and added to the system.

You will be given a login and password for our online booking and medication request system.

You will also have access to your medical records for all further consultations and investigations. To gain access to your previous records, you will need to complete the form provided at registration.

Please note that all patients will be registered with Stokewood Surgery but you may opt to be seen in Old Anchor Surgery if the services you require are available. These include routine GP, Nurse, and Healthcare Assistant appointments.
