If you would like to submit your blood pressure readings, please either print and complete the "Home BP Readings Chart" below. You can then either post/email/bring in the results to Stokewood for us to save to your records.

Alternatively, you can input your readings into the "Home BP Calculator" and email the results to hiowicb-hsi.stokewoodsurgery@nhs.net.

There is also a NEW way to submit BP Readings - please press the "Home BP Readings Online Form" button below to submit readings online.
You will need at minimum 5 days of consecutive readings (2x morning, 2x evening) in order to complete the form.
