Once completed please bring in the registration form to Stokewood Surgery and include the requested ID, for the form to be authorised by a member of our Care Navigation Team. 

ID required when submitting this form
. Parent/Guardian: 1x Proof of Address (Utility Bill, Bank Statement) and 1x Photo ID (Driving Licence, Passport)
. Child: Birth Certificate

It is not mandatory for the child to attend an identification check, only the Parent/Guardian as long as they bring in all required documents.
